dc.description.abstractThis research developed a wireframe representation of a prototype Clinical Decision Support Tool that enables the comprehensive, efficient and efficacious management of patients with Cardiometabolic Disorder and Diabetes Mellitus Type 2. This research took place in the Eastside Health Network, an Accountable Care Organization and employed user-centered, iterative design principles to create both the user interface and the backend decision support logic. The design process took place in the context of a cross-functional team of physicians, pharmacists, diabetic nurse educators, care managers and administrators.
